AI Hallucination Prevention
A confident wrong answer is more dangerous than no answer at all. Here is how to catch the bad ones before they become decisions.
Hallucination is not a model bug. It is the natural consequence of how language models work. The job is not to eliminate it. The job is to prevent it from becoming an action.
Every model in use today produces some rate of confident-but-wrong outputs. The rate varies. The variance does not. Treating that as a model-quality problem is a category error. Even the best model on the best benchmark hallucinates often enough that any production deployment without a containment layer will eventually post a wrong number to a real customer. The question is whether you find out from the model logs or from the customer.
The best teams have stopped trying to make their models perfect and started designing systems where hallucination is contained. The model proposes. A separate layer verifies. Anything that cannot be backed by a citation in the trusted source of truth either gets blocked or gets escalated. The model still hallucinates at the same rate it always did. The customer never sees it.
The three containment patterns that work
First, citation-required outputs: if the model claims a fact, it has to point at the document it came from. Outputs without citations are treated as low confidence by default. Second, action gating: the high-stakes decisions (refunds, commitments, policy claims) cannot be executed by the model. They are proposed by the model and approved by a separate policy engine that checks the proposal against the rules. Third, drift monitoring: the system tracks the rate at which the model's outputs match the ground truth, and raises a flag when it changes.
None of these are model improvements. All of them are system design. That is the point.

What is an AI hallucination?
An AI hallucination is a confident output that has no basis in the underlying data the model was supposed to ground itself in. It is not a bug in the model. It is a property of how language models work: they predict plausible next tokens, not true ones. The risk is not that the model is wrong. The risk is that it sounds right.
Why do AI hallucinations matter for enterprise?
Because the cost is not the bad output. It is the action taken on the bad output. A hallucinated refund amount that posts to a P&L. A hallucinated policy that goes to a regulator. A hallucinated commitment that lands in a customer's inbox. The output is the symptom. The decision is the damage.
Can hallucinations be prevented entirely?
Not at the model layer. Models will continue to produce some rate of confident-but-wrong outputs. What can be prevented is the propagation: catching the output before it becomes an action, requiring a verifiable source, and refusing to act on claims that cannot be cited. This is a system design choice, not a model choice.
What is the difference between hallucination prevention and hallucination detection?
Detection finds the bad output after the fact. Prevention stops the bad output from becoming a bad action. Detection is useful for measurement and improvement. Prevention is what protects the customer and the balance sheet. The two work together: detection feeds the prevention layer's policies.
